Findings
CR – Upright AP view demonstrates an impaction fracture with contour deformity of the superolateral humeral head. The glenohumeral relationship is normal. Fat-fluid level in shoulder joint (subacromial space) indicates an acute injury.
Discussion
Contour deformity of the posterolateral humeral head occurs from impaction of the humerus against the anterior labrum during anterior subcoracoid shoulder dislocation. The presence of a fat-fluid level in a joint indicates leakage of marrow fat as a result of an intra-articular fracture.
